Your referral coordinator has an office list. The useful question is whether those offices still operate where the list says they do. A changed practice address deserves a check before another directory update or relationship visit. Public provider records offer a starting point. Your clinic supplies the territory, relevant disciplines, existing office relationships, and corrections. A scheduled comparison turns those inputs into a short verification queue. This page concerns public professional-office information. Patient records are outside the proposed function. ## The flagship tool: referral-office change watch. **Your question:** “Which offices near us changed, and which relationships should our coordinator verify this week?” The function compares saved observations from the NPPES NPI Registry. It identifies a changed location or taxonomy and shows the earlier value beside the latest one. Staff decide whether that change describes a move, an additional office, or a correction. ### What a public record says. The [NPPES API](https://npiregistry.cms.hhs.gov/api/?version=2.1&state=NC&city=RALEIGH&taxonomy_description=Physical%20Therapist&limit=1) returned a physical-therapist record during our 2026.10.27 check. Its fields included an NPI, taxonomy, update information, and addresses distinguished as mailing and practice locations. Those distinctions matter. A mailing-address edit should not become a practice-move alert. An enumeration date should not become a grand-opening date. It must remain separate from the registry's dates. A record newly found by your collector may describe a long-established office. ### How the function builds the queue. The practice owner chooses the cities, professional disciplines, and office types to watch. The collector takes a bounded weekly snapshot and preserves its coverage and retrieval date. Incomplete results remain visibly incomplete. The matching step uses NPI first. It then compares normalized practice addresses and approved taxonomy fields. Records sharing an address remain distinct. A medical building can contain several practices, and one organization can have several locations. The clinic's own office roster supplies relationship context. Staff can mark an established office, an unresolved business location, or an office outside the service territory. No patient referral history is needed for this first version. A missing record creates a verification task. It does not establish a closure. The function requires another observation before marking a persistent disappearance, then asks staff to confirm the business fact. ### What staff receive. The weekly queue shows the earlier and current business facts, observation dates, source link, relationship label, and reason for review. Each row has a place for the coordinator's confirmed answer. Useful review outcomes include a confirmed move, an additional location, a mailing-only correction, a duplicate, and an unresolved change. These are workflow categories selected for the build. They are not classifications supplied by NPPES. The first run establishes a baseline. It cannot honestly report changes without an earlier comparable observation. A failed retrieval produces a source-failure note and preserves the last usable snapshot. **Who directs and who checks:** the practice owner chooses the territory and disciplines. The referral coordinator confirms office facts. A licensed clinician judges whether a professional discipline belongs in the clinic's relationship research. **Why it compounds:** each confirmed move, rejected match, and organization alias improves the office record. A new coordinator inherits that history. Another clinic can retrieve the public registry, but it cannot retrieve your staff's corrections and relationship decisions. The starting scope is one working function for snapshots, comparisons, and the review queue. A separate approved directory integration can become a second function. ## Two more tools. ### Local service-mix planning table. “Which service categories deserve a capacity study before we change staffing?” The CMS Medicare Physician and Other Practitioners by Provider and Service dataset supports a historical planning table. Provider-service rows include NPI, specialty, ZIP, procedure code, place of service, service counts, and payment fields. The clinic selects relevant service groups and compares compatible years and settings. The table must retain suppressed and missing values as unknown. Unique-beneficiary counts cannot be added across procedures to create a patient count. Medicare observations do not establish all-payer demand, referral flow, clinical need, or a competitor's financial condition. A clinical director defines the service categories. A billing reviewer checks interpretation. The owner compares the historical evidence with the clinic's own aggregate capacity record before considering a change. ### Coverage-document deadline watch. “Which published policies should our billing team review before their effective dates?” The [Medicare Coverage Database downloads](https://www.cms.gov/medicare-coverage-database/downloads/downloads.aspx) provide a starting document register. A metadata-only function keeps document identifiers, versions, jurisdictions, dates, and official links. It separates proposed, future-effective, effective, and retired documents. Billing staff record what a reviewed change means for office procedures. The function does not decide coverage or change a code. Changed-office counts require comparable stored observations and a method that distinguishes identity changes from shifts in the search results. A baseline must remain labeled as a baseline. The public view excludes practitioner names, home addresses, phones, and authorized-official details. The private build adds the clinic's approved business-office roster and staff verification history. Neither needs patient information or a record of a visitor's health interests. ## The limits that shape the design. Registry presence cannot prove that an office is open, accepting referrals, appropriately licensed, or clinically suitable. Confirm those facts through the relevant business and professional checks. The proposed queue supports office research. It does not direct patients, reward referrals, send messages, or rank people for employment or insurance decisions. Any later contact workflow needs its own approved recipients, wording, and channel rules. NPPES automation terms and production request limits remain a launch check. A successful small API request proves access to that response. It does not prove unlimited collection rights or complete city coverage.
