List the questions it asks, where answers go, which vendors receive them, and who reads the message. Keep public inquiries separate from the practice's approved patient intake process. Start with the smallest public contact task your office needs. Have the privacy reviewer decide what belongs there before adding fields or tracking tools. ## Establish your requirements first HIPAA applies to covered entities and business associates as defined by the rules. HHS identifies the electronic-transaction condition for covered healthcare providers. Have the responsible reviewer determine the practice's status and applicable requirements. Do not describe a website as compliant because of a lock icon or disclaimer. ## Inventory every destination Ask the builder to identify the form service, email destination, stored submissions, analytics, embedded widgets, and error logs. Record which receives field values, page addresses, identifiers, or another data category. Its bulletin notes a court vacated the provision concerning an IP address linked to visiting an unauthenticated public page about conditions or providers. Avoid declaring every public page view a HIPAA disclosure. Have your reviewer assess the actual information and recipients. Record unresolved tracking behavior as a review item. ## Give each field a purpose Name the office task served by each proposed field. Remove a field from the draft when its purpose is unclear. Keep detailed histories and completed clinical forms within the approved intake route. W3C recommends labels that identify form controls and are associated with them.
