Confirm that each blank form is current, understandable, and connected to the practice's approved return process. A working download button is only the first check. Start with public blank documents and instructions. Keep completed patient paperwork out of the website content inventory and demonstration files. ## Inventory documents by their real purpose Ask the front-desk lead for the current required-form list. Compare it with every public download, including old links from provider pages and welcome messages. Open the file and compare its contents with the document owner's source. ## Explain the download before the visitor opens it Use a descriptive link label that identifies the form. State the file type and explain whether the visitor should print it, complete it digitally, or follow another approved process. Verify any file-size label against the final file. Keep essential preparation instructions on the webpage as well as in the download. Give visitors the approved contact route for document-access or completion questions. Avoid asking them to send completed forms through a general website inquiry box. If a portal is the approved route, identify that handoff clearly. Confirm the destination with the practice before publishing it. Ask the document specialist to inspect the actual published file. Have them check instructions, headings, questions, and response areas in their intended sequence. For interactive forms, include the field order and labels in the requested review. Keep a readable text extraction as one piece of evidence. Do not accept it as a complete document-accessibility assessment. Record the checks performed and the issues that remain unresolved. ## Test the full public download path Open the paperwork page on a phone and a computer. Follow each link and inspect the returned file. Confirm its title, version, page count, and relevant location against the approved inventory. Use synthetic entries if the practice authorizes a completion demonstration. Check the proposed save or print instructions. Stop before sending anything into a real patient record or public inquiry channel. Review the completed-form route separately with the responsible staff. A successful file download does not establish that a completed form reaches the correct team. ## Replace old versions as a complete change Ask the builder to list pages and messages that reference a retired document. Have the practice approve what happens to its old URL. Recheck linked instructions whenever the replacement changes required steps. Save the approved source document, public file, and test results together. Assign one office role to notify the maintainer when paperwork changes.
