# Proposed and final physician fee schedule rules: prepare an outpatient policy review

Start an outpatient policy review by identifying the document in front of you. Record its rule identifier, calendar year, publication date, document type, and source link. Then ask the billing owner which passage needs a practice-specific decision.

This preparation keeps a proposal, a published final rule, and an office instruction in separate records. Your review sheet should show the evidence connecting them. A saved headline alone gives the next reviewer too little to check.

The [CMS physician fee schedule regulation index](https://www.cms.gov/medicare/payment/fee-schedules/physician/federal-regulation-notices) lists display dates, publication dates, and calendar years. Use it as the starting directory. Open each selected entry and its linked document before recording status or timing.

The worksheet below is an editorial method for organizing that reading. It supplies questions for the responsible reviewer. It does not calculate fees, determine coverage, interpret procedure descriptions, or authorize billing changes.

## Separate the document from the decision

Make two records at the start. The first describes what the publisher issued. The second describes what your practice has decided.

The publication record needs an exact identifier and a link. The decision record needs a question, an owner, and an approval state. Connect the records without treating either as a substitute for the other.

For example, an administrator might locate a final rule and ask whether a staff reference sheet needs review. That is a valid research handoff. It does not establish that the reference sheet is wrong, that every service is affected, or that staff should change their work immediately.

Use explicit states such as collected, awaiting review, clarification requested, and approved for the named internal use. Define these as office workflow labels. Do not present them as statuses assigned by CMS.

Keep a separate field for the source's document type. Otherwise, an internal label such as reviewed could obscure whether the underlying publication was proposed or final.

## Read a completed historical pair

Use calendar year 2026 as a dated example. This guide does not predict a later year's final rule.

| Publication field | Proposed entry | Final entry |
|---|---|---|
| CMS identifier | CMS-1832-P | CMS-1832-F |
| Regulation type | Proposed Rule | Final Rule |
| Calendar year | 2026 | 2026 |
| Display date | July 14, 2025 | October 31, 2025 |
| Publication date | July 16, 2025 | November 5, 2025 |

These fields come from the [CMS-1832-P entry](https://www.cms.gov/medicare/payment/fee-schedules/physician/federal-regulation-notices/cms-1832-p) and [CMS-1832-F entry](https://www.cms.gov/medicare/payment/fee-schedules/physician/federal-regulation-notices/cms-1832-f), checked October 28, 2026.

The controlling proposed document identifies its action as proposed rule. Its DATES section gives a September 12, 2025 comment deadline. The final document identifies its action as final rule and states a January 1, 2026 effective date. See [Federal Register (FR) document 2025-13271, opening and DATES](https://www.govinfo.gov/content/pkg/FR-2025-07-16/html/2025-13271.htm) and [FR document 2025-19787, opening and DATES](https://www.govinfo.gov/content/pkg/FR-2025-11-05/html/2025-19787.htm).

Notice the different questions those dates answer. Publication identifies an edition. A comment deadline identifies a submission window. An effective-date statement needs its own source field. Keep the text's scope beside it instead of spreading one date across every office task.

This historical pair supplies a useful training exercise. Ask a colleague to find the two action labels and the final document's DATES section independently. Compare the records before discussing any practice policy.

## Capture enough identity to reopen the evidence

Create one row per publication. Avoid replacing the proposed row when you find the final row. The earlier record explains where an initial planning question came from.

Use the following proposed field set:

| Field | What the administrator records |
|---|---|
| Rule identifier | Exact CMS entry identifier, including suffix |
| Calendar year | Publisher's stated year, with field source |
| Document type | CMS label and controlling document action |
| Publication date | Date attached to the selected published edition |
| Display date | Separately labeled CMS field |
| Controlling document | Direct link and FR document number |
| Passage locator | Page, section, heading, and paragraph where available |
| Access date | Date the administrator opened the evidence |
| Review question | One bounded question about one office workflow |
| Approval owner | Role that can resolve that question |

When a field is unavailable, write not established and assign a follow-up. Do not copy a neighboring date into the empty cell. A visible gap gives the billing owner something concrete to resolve.

If you retain a local document, give it an edition-specific name. Preserve the source URL with it. Keep your own notes outside the source copy so another reviewer can distinguish publisher text from office interpretation.

## Compare passages before describing a policy change

Choose one question before comparing a long proposed document with a long final document. An unbounded request to summarize everything leaves the approval owner without a clear decision.

Write the question in operational terms: does the office's approved training reference need revision, and which source passage should the reviewer inspect? Avoid starting with a proposed answer.

Find the relevant heading in each edition. Save the locator even when the section titles resemble each other. Read enough surrounding text to understand what the passage addresses. Then ask the billing owner to confirm the comparison's relevance.

Do not assume that matching words establish matching requirements. Likewise, a different paragraph number is a reason to investigate, rather than evidence of a changed office obligation.

Use a comparison table with proposed passage, final passage, observed difference, unresolved meaning, and decision owner. Keep the observed difference descriptive. Reserve conclusions about application to the practice for the qualified reviewer.

Where an attachment matters, identify that attachment separately. Record its displayed name and edition information. Do not let a folder called final become the only evidence of which file someone read.

## Work through a synthetic office example

The following example is fictional. It demonstrates review preparation without asserting that a particular outpatient service or practice is affected.

An administrator finds an old planning note titled annual staff reference review. Its source points to the historical proposed entry. The office has not recorded whether the planning question was later resolved.

The administrator creates review item POLICY-EXAMPLE-01. Its subject is the evidence behind the staff reference, rather than a payment amount or claim decision.

| Review element | Synthetic entry |
|---|---|
| Existing office artifact | Annual staff reference, internal edition A |
| Source attached to old note | CMS-1832-P entry |
| Additional evidence collected | CMS-1832-F entry and controlling final document |
| Question | Which passage should govern the next reference review? |
| Unresolved detail | Practice-specific relevance of the selected passage |
| Decision owner | Billing policy owner |
| Proposed state | Awaiting owner review |
| Authorized office change | None recorded |

The administrator attaches both source rows and the existing internal reference. The billing owner receives the exact question and document links. The request does not ask the owner to infer which attachment or staff sheet started the inquiry.

If the owner needs another specialist, record that referral and the narrower question. Keep the review open until the designated owner records a disposition. Do not turn silence into approval.

If the owner decides no reference change is needed, retain that decision with its reason and evidence. A later administrator should be able to distinguish a completed review from a forgotten task.

## Prepare the review meeting around workflow questions

Organize the meeting by the office artifacts that might need attention. Ask which approved staff reference, internal checklist, or training note is in scope. Request the current edition before proposing edits.

Ask who owns the meaning of the selected passage. The administrator can assemble the documents. The billing owner should identify any additional payer, compliance, or clinical review needed for the question.

Then ask what would count as a completed decision. A useful answer names the artifact, the approved wording or no-change result, and the person authorized to release it internally.

Keep publication research and implementation tasks in separate parts of the agenda. Document review may end with another question. Implementation needs an explicit decision and its own scope.

Record the smallest next step. That might be locating a referenced attachment, checking a passage against another official document, or asking a qualified owner to resolve timing. Avoid assigning an office-wide instruction before that evidence exists.

## Use a reusable approval worksheet

Copy these prompts into your existing policy review process. Treat them as suggested preparation fields rather than a mandated CMS form.

1. Which exact publication started this review?
2. Which calendar year and document type does its entry state?
3. Where is the controlling document, and which passage matters?
4. Which dates were sourced separately, with their labels preserved?
5. Which current internal artifact depends on the passage?
6. What question needs an authorized decision?
7. Who owns that decision, and who else must review it?
8. What evidence is missing or contradictory?
9. What disposition did the owner record?
10. Which internal edition, if any, was approved for release?

Before closing the item, ask someone other than the preparer to reopen the links and locate the passage. Confirm that the worksheet still identifies the same edition. Save any correction to the worksheet with a reason.

For a publication that corrects an earlier document, continue with [tracing physician fee schedule correction notices](/articles/outpatient-pfs-correction-notice-version-review). That guide addresses version identity and date differences that a proposed-versus-final comparison alone cannot settle.

## Keep the research history with the office record

Maintain the question, source rows, approval, and resulting internal edition together. The [business memory guide](/articles/business-memory-for-ai-agents-guide) explains the wider value of keeping decisions in files the business controls.

For this workflow, a useful retained record answers a concrete question: why does this staff reference say what it says? Preserve enough evidence for a new owner to answer without reconstructing the review from scattered messages.

The [therapy and outpatient health overview](/for/therapy-and-outpatient-health) provides the broader office context. Its [industry intelligence guide](/intelligence/therapy-and-outpatient-health) describes a publication watch that can feed this review process. Keep the watch's observations separate from the owner's decisions.

Bring one sample publication record, one internal reference, and the approval roles when defining work with Scale With Search. Those materials make the proposed document and review process concrete.
