# Physician fee schedule corrections: trace the changed text before updating office instructions

Treat a physician fee schedule correction as a document relationship to investigate. Find the original rule, locate the changed passage, and record the correction's own identity and date language. Then ask the billing owner whether an internal instruction depends on that passage.

An administrator's useful output is a traceable review record. It should connect a publisher's change to a specific office question. It should also show when the connection remains unproven.

The [CMS physician fee schedule regulation index](https://www.cms.gov/medicare/payment/fee-schedules/physician/federal-regulation-notices) provides a directory for this work. Preserve its separate entries while following their document links. Do not collapse a rule and later corrections into one undated file.

This guide uses historical calendar year 2026 publications. The proposed workflow organizes source evidence for review. It does not determine practice-specific payment, coverage, coding, or claim treatment.

## Establish the original rule before reading the correction

Start with a source row for the rule that was corrected. Record the rule identifier, Federal Register (FR) document number, publication citation, and direct document link. Then create another row for each correction you investigate.

The CMS entry for [CMS-1832-F](https://www.cms.gov/medicare/payment/fee-schedules/physician/federal-regulation-notices/cms-1832-f) identifies a final rule for calendar year 2026. Its controlling publication is [FR document 2025-19787, November 5, 2025, 90 FR 49266](https://www.govinfo.gov/content/pkg/FR-2025-11-05/html/2025-19787.htm).

Use that combination as the parent identity in this example. A similar title alone is too weak for an office record. Require the correction's own text to identify the publication it changes.

Build a relationship table with parent document, correction document, publisher-stated connection, affected passage, and review result. Leave the affected-passage field unresolved until someone opens the text.

If your starting question concerns whether a document is proposed or final, begin with the [outpatient proposed and final rule review guide](/articles/outpatient-pfs-proposed-final-rule-review). Establishing status and tracing corrections are related reading tasks, with different worksheets.

## Preserve both identifiers when labels differ

The [CMS-1832-CN3 entry](https://www.cms.gov/medicare/payment/fee-schedules/physician/federal-regulation-notices/cms-1832-cn3) labels its regulation type as final rule; correction. Its linked March 12, 2026 document carries the identifier CMS-1832-F2 and the action correcting amendments. Its FR document number is 2026-04797. See the [controlling publication's opening](https://www.govinfo.gov/content/pkg/FR-2026-03-12/html/2026-04797.htm).

Store the CMS entry label and document identifier in separate fields. Do not silently rename either to make the record look consistent. Add the link that connects them and an access date.

That approach lets another reviewer repeat the lookup even when they start from a different label. It also prevents an office search from creating two unrelated review tasks for the same linked publication.

Use explicit field names: CMS entry identifier, printed document identifier, and FR document number. A single field called rule number forces the preparer to discard information.

Check the parent's identity inside the correction before marking the relationship confirmed. If that evidence is absent or conflicting, preserve the question and request review. Do not match documents solely because their identifier stems resemble each other.

## Keep publication, display, effective, and applicability dates apart

The March CMS entry gives March 12, 2026 for both display and publication. The controlling document's DATES section states an effective date of March 12, 2026 and applicability to January 1, 2026. Section I, Background, says the correcting amendment's provisions are effective January 1, 2026. Preserve that difference for the billing owner. [CMS entry fields](https://www.cms.gov/medicare/payment/fee-schedules/physician/federal-regulation-notices/cms-1832-cn3); [FR document 2026-04797, DATES and Background](https://www.govinfo.gov/content/pkg/FR-2026-03-12/html/2026-04797.htm).

| Proposed worksheet field | How to record it |
|---|---|
| Display date | CMS field value and source link |
| Publication date | Published edition's date and citation |
| Effective date statement | Exact location and separately recorded wording |
| Applicability statement | Exact location and separately recorded wording |
| Other date language | Conflicting or qualifying passage, with locator |
| Owner's disposition | Interpretation, evidence, and authorized scope |
| Internal release date | Office approval record, kept apart from source dates |

Do not resolve a date conflict by choosing the earliest date or the most prominent heading. Give the responsible owner both passages. Ask what further evidence is needed before an internal instruction changes.

Your access date belongs in another field. It records when your office checked the source. It cannot explain the publisher's intended timing.

Likewise, a publication date should not automatically become a staff implementation deadline. Require an owner-approved action record for that separate decision.

## Trace the November correction as its own entry

The historical sequence also includes CMS-1832-CN2. Its [November 28, 2025 publication, FR document 2025-21458](https://www.govinfo.gov/content/pkg/FR-2025-11-28/html/2025-21458.htm), identifies technical corrections to the November 5 final rule. Its DATES section gives November 28, 2025 as the correcting document's effective date.

Record this as another linked publication, rather than replacing the final rule's publication date. Its identity and changed passages belong to its own row.

When reviewing several corrections, ask whether each affects the passage your office relies on. Avoid assuming that a later notice republishes or resolves every earlier issue. Trace each relevant instruction through its own cited locations.

Use the index to identify candidate documents, then use the text to establish relationships. The directory helps you find evidence. The review record should show which evidence someone read.

## Locate a changed passage without reproducing procedure descriptions

A bounded example is a wording correction in the March publication. Section IV.A.4 directs a change on original page 49540, first column, third full paragraph, lines 6 through 8. It changes the verb from “will” to “may” in a manufacturer-contact statement. [Correction instruction IV.A.4](https://www.govinfo.gov/content/pkg/FR-2026-03-12/html/2026-04797.htm).

Open [original page 49540 in FR document 2025-19787](https://www.govinfo.gov/content/pkg/FR-2025-11-05/html/2025-19787.htm) and locate that paragraph. The example demonstrates passage matching. It makes no assertion that the passage affects your outpatient practice.

Preserve both locations in the review record. Label one original passage and the other correction instruction. A browser search result without the surrounding page reference is insufficient for the handoff.

Keep the extracted difference short. The purpose is to identify the changed wording for the owner. You do not need to reproduce procedure descriptions or copy a large section into an office spreadsheet.

If the original passage cannot be located, stop that comparison at unresolved. Save the attempted locator and the document edition. Ask for help finding the source instead of inventing a substitute passage.

## Build a synthetic office-instruction change record

The following example is fictional. Its office artifact exists only to demonstrate the review method. It does not describe a real practice or a required policy change.

Suppose an internal research checklist asks whether a publisher describes its own intended action as definite or discretionary. A preparer selects the manufacturer-contact correction above as a training example.

| Record field | Synthetic value |
|---|---|
| Internal item | TRAINING-EXAMPLE-02 |
| Office artifact | Source-reading checklist, edition A |
| Original evidence | FR document 2025-19787, page 49540 |
| Correction evidence | FR document 2026-04797, IV.A.4 |
| Observed text difference | Contact verb changes from will to may |
| Proposed office edit | Add this pair to a document-reading exercise |
| Practice-policy relevance | Not established |
| Timing question | Owner must review the document's date statements |
| Approval owner | Billing policy owner |
| Claims or patient-facing action | None proposed |

The preparer submits the record with both links and the existing checklist. The owner can approve the training example, reject it, or request clarification. Approval of that example would concern the training artifact only.

If the owner wants to examine another office instruction, create a separate question with that instruction's exact edition. Do not extend the example's approval to unrelated policies.

After a decision, retain the proposed wording and final disposition. Record no change required when appropriate. A complete review history should explain a deliberate no-change result as clearly as an approved revision.

## Preserve an unresolved comparison without losing the work

A review can produce useful evidence even when the owner cannot decide applicability immediately. Keep the verified document relationship separate from the unanswered office question.

For example, your preparer might confirm the parent publication and locate the changed paragraph. The owner might still need clarification about timing. Mark the passage match complete and the timing question unresolved.

Assign the open question to one role. Record the evidence requested and the next review date. Avoid a generic waiting label that leaves nobody responsible for returning to it.

When clarification arrives, attach its source and date to the same item. Preserve the earlier question so the record explains why implementation waited. Give the owner the revised package for a fresh disposition.

If an attachment changes during that interval, retain its earlier edition information. Record the new observation separately and ask whether the comparison needs reopening. Do not overwrite the evidence that supported the original review.

This structure also supports a deliberate rejection. Record why the passage does not support the proposed office edit. Keep that reasoning beside the rejected wording so another preparer can understand the decision.

## Define the handoff to the billing owner

Send a review package with a bounded question, rather than an instruction to update everything. Identify the internal artifact, its source dependency, the changed passage, and the unresolved date issue.

Ask the billing owner to state whether the cited passage is relevant. Ask which additional reviewer or official clarification is needed if applicability remains uncertain. Keep that uncertainty visible in the task state.

Require a separate approval before releasing revised office instructions. Record the approved edition and intended internal audience. Keep training approval, billing policy approval, and public communication approval as distinct decisions.

Do not attach patient records to this publication review. Use empty templates and fictional examples for the document comparison. The evidence question concerns published text and internal wording.

If a later review raises a claim-specific question, route it through the practice's authorized process. Keep that separate from this metadata and version worksheet.

## Close the review with a reproducible record

Before closing, have another person reopen the original and correction links. Ask them to identify the same passage and explain which date fields remain unresolved. Correct the worksheet if they cannot reproduce the trace.

Retain the parent document, correction row, office question, owner decision, and any approved internal edition together. The [business memory guide](/articles/business-memory-for-ai-agents-guide) explains how owned records preserve that chain for a future reviewer.

For the broader workflow, use the [therapy and outpatient health hub](/for/therapy-and-outpatient-health) and its [industry intelligence overview](/intelligence/therapy-and-outpatient-health). A publication watch can surface a candidate correction. The owner still needs a document review before office instructions change.

Bring a sample correction and one internal reference when discussing the workflow with Scale With Search. Define the source fields, review roles, unresolved states, and retained approval record in the written scope.
