# NPPES dates: separate enumeration, last update, and first observation before calling a practice new

A recent date beside an organization deserves a precise label before it enters your medical billing research list. Ask which event it describes. An NPI assignment, a registry update, and your first observation answer three different questions. Keep all three rather than choosing whichever makes the organization look newest.

For a billing-company owner, the useful decision is which practice deserves further research and why. That decision needs a stated evidence gap. The [medical billing company overview](/for/medical-billing) explains the office-work boundaries. This guide addresses the dates behind a public practice research list.

Use the worksheet below as a proposed research procedure. It does not establish any organization's opening, enrollment, operating status, or need for a billing service. Each conclusion needs its own supporting evidence.

## Label the three clocks before sorting

The [NPPES API field mappings](https://npiregistry.cms.hhs.gov/help-api/json-conversion) map Provider Enumeration Date to `basic.enumeration_date` and Last Update Date to `basic.last_updated`. The [registry details help](https://npiregistry.cms.hhs.gov/help/help-details) defines enumeration as NPI assignment and last update as the NPI's latest update. Both sources were reopened October 28, 2026.

A bounded organization-only API check that day returned both fields under `basic` for an `NPI-2` result. That check establishes field availability in the inspected response. It is not a count of new practices or a territory coverage assessment. [Inspected CMS API query, limited to one result](https://npiregistry.cms.hhs.gov/api/?version=2.1&enumeration_type=NPI-2&postal_code=27101&limit=1).

Define **first observation** as the earliest dated observation your own retained research record supports. This is your research field, not a substitute name for either CMS date. If your research starts today, its history starts today. An old enumeration date does not give your company an earlier observation history.

| Date label | Question it answers | What to record beside it |
| --- | --- | --- |
| Enumeration date | When does the registry say this NPI was assigned? | Exact source field and retrieval date |
| Last-update date | What latest update date does the registry show? | Current value and any retained comparison |
| First observation | When did our retained record first capture this organization? | Observation date, coverage, and source reference |

Use the first two labels consistently in reports. Use first observation only when you can locate the underlying record. A colleague remembering that a name looked familiar is a research clue, not a dated observation.

## A fictional organization with three different dates

Consider the fictional Harbor Lantern Therapy Group in Greensboro. The example uses invented dates and a city only. It describes no actual provider, opening, or business relationship.

Suppose its research card contains an enumeration date of May 12, 2022. The current registry observation shows a last-update date of October 20, 2026. Your company's first retained observation is October 28, 2026. You also see a specialty entry that deserves checking against your approved service categories.

The defensible initial note reads: “First observed in our research on October 28. Registry enumeration is dated May 12, 2022. Latest registry update is dated October 20, 2026.” The opening date remains unknown.

Do not describe the specialty as newly added unless you have a comparable earlier record showing its absence. A current value plus an update date gives you no before-and-after comparison for that field. Write “current specialty requires fit review” instead.

Now suppose your researcher locates an approved earlier observation dated October 14. It shows a different practice city. Preserve that observation and compare the same address role. The revised note can identify a difference between two observations. It still needs evidence of what happened operationally and when.

The earlier retained observation also changes the first-observation field. Record the correction and its reason. Do not silently replace the date and erase why the weekly report changed. The next reader needs both the corrected value and a traceable explanation.

## Treat an update date as a prompt to compare

CMS-10114 separates an initial application from a change of information. That distinction supports keeping assignment and later record changes separate. [CMS-10114, revision July 2026, page 1, section 1A](https://www.cms.gov/medicare/cms-forms/cms-forms/downloads/cms10114.pdf).

Your research procedure should then ask what evidence shows the particular change. Compare the current observation with the last successful comparable observation. Name the field, prior value, present value, and dates of both observations. If the earlier record is missing, state that limitation in the comparison column.

For a taxonomy difference, ask the service lead whether the recorded classification fits work the company accepts. Do not turn the classification into a claim about current case mix, payer arrangements, or available billing work. Those questions require other evidence.

For an address difference, compare mailing with mailing and practice location with practice location. A change between unlike roles can manufacture a move that your observations never established. Use the companion guide to [organization mailing addresses, practice locations, and subparts](/articles/nppes-organization-mailing-practice-location-subparts) before making that comparison.

Where several fields differ, list them separately. Avoid a single narrative such as “new practice expanded and changed specialties.” That sentence compresses multiple unresolved questions into one apparent event. A field comparison lets the reviewer accept one observation while holding another conclusion open.

## Make first observation honest about coverage

Before starting a watch, write down the territory, specialty criteria, address role, and intended observation schedule. Keep a record of completed checks. These are recommended boundaries for your research, not evidence that a particular collection system is running.

For example, suppose your approved research covers one city and two specialty categories. On the first completed day, label every result “first observed within this research scope.” Do not call the same list “practices opened this week.” Your monitoring start date measures your own work.

If the next check fails, record a failed check. Do not treat the empty output as evidence that organizations closed or left the territory. Resume with the next successful observation and describe the gap. Keep the last successful comparison date visible.

A scope expansion needs the same care. Adding another city may introduce an organization with a much older enumeration date. Label the reason it entered your list: added geography, changed specialty criteria, or first successful observation. Keep that reason separate from its registry dates.

When reporting totals, state the unit and interval. “Organizations first observed in our completed checks” is a different measure from “organizations assigned NPIs during this period.” Neither label should become a practice-opening count without opening evidence for each included organization.

## Give opening, operation, and service fit separate evidence

Create three conclusion fields rather than a single qualified-lead label. This makes the research useful even when the evidence supports only one question. An organization can deserve a second look while its opening date remains unresolved.

For **opening evidence**, record the exact public statement you found, its publisher, the date it describes, and your access date. A hypothetical practice announcement that says “opening next month” supports a planned opening. Your note should preserve the future tense until later evidence supports a different conclusion.

For **operating evidence**, write the specific fact the source supports. A practice's own current location page may describe a service at a city. If that is your evidence, record that description and the checked date. Do not quietly promote it to proof of all locations, current capacity, or payer participation.

For **service fit**, use your company's approved scope. Ask which business task is relevant, what specialty evidence supports the match, and what remains unknown. Do not infer financial weakness or a wish to replace an incumbent provider from a recent registry update.

Where sources disagree, preserve both references and name the question for review. Give the question an owner and a next step. The objective is a readable decision record, not a uniformly green research list.

## A research note your colleague can review

The following fictional note continues the Harbor Lantern example. The labels are proposed worksheet fields. They are not additional NPPES fields or claims about an actual organization.

| Worksheet field | Fictional entry |
| --- | --- |
| Record identity | Harbor Lantern Therapy Group; organizational record reference retained privately |
| Registry dates | Enumerated May 12, 2022; updated October 20, 2026 |
| First retained observation | October 14, 2026, after locating the earlier research record |
| Current observation | October 28, 2026; approved city and specialty checks completed |
| Observed difference | Primary practice city differs between comparable observations |
| Opening evidence | None reviewed; opening date unknown |
| Operating evidence | Practice page review pending; no conclusion entered |
| Service fit | Specialty comparison pending with the service lead |
| Reviewer decision | Hold any opening label; investigate the location difference |

Keep the underlying source references with the note. A table copied into a weekly email should not become the only record. Your colleague must be able to inspect what the researcher saw and distinguish that observation from the later decision.

The [public practice signals overview](/intelligence/medical-billing) places this note within a proposed registry watch. For the durable record structure, use the [business memory guide](/articles/business-memory-for-ai-agents-guide). Keep accepted corrections and reviewed conclusions under your company's control.

## Review the headline before sharing the list

Ask a colleague to read the report heading without seeing the worksheet. Have them explain what “new” means. If they interpret it as newly opened practices, replace the heading with the measured event. “First observed organizations” or “registry records with recent enumeration dates” states a narrower, checkable result.

Then test the procedure with the fictional example. The reviewer should preserve the old enumeration date, identify the recent update, and explain the corrected first observation. They should leave the opening question unresolved. A procedure that invents an opening from those dates needs revision before real business research.

Retain one accepted example and one held example as staff reading exercises. Require the same labels on later reports. Change the instructions when reviewers discover ambiguity, and record why the change was accepted.

Scale With Search can scope a research handoff around the territory, source labels, retained observations, and named reviewer. Bring the business question and the record your colleague needs to check.
