# Device registration and clearance: request separate evidence before a med spa purchase

Before reviewing a device purchase, request separate establishment registration and device listing evidence. Request the applicable marketing authorization or exemption basis. Put each document in its own category. Record exactly which purchasing question it addresses.

A supplier packet can contain several impressive attachments and still leave the device identity unresolved. The first review task is document sorting. The next task is asking for missing identifiers. Neither task determines clinical suitability or authorizes a purchase.

This guide proposes a purchasing evidence checklist for a med spa owner or purchasing lead. Your medical director and regulatory reviewer interpret the documents within their authority. Commercial terms and the final decision remain with the people responsible for the transaction.

## Start with three different record types

FDA describes establishment registration as an annual process for covered device establishments. Device listing generally identifies their devices and activities. Where marketing authorization is required, the registration and listing process also calls for the relevant premarket submission number. [FDA: Device registration and listing, opening explanation, checked 2026.10.28](https://www.fda.gov/medical-devices/how-study-and-market-your-device/device-registration-and-listing).

FDA separately states that registration and listing do not establish approval, clearance, or authorization. The agency does not issue device-facility registration certificates. [FDA: Important reminders about registration and listing, first reminder, checked 2026.10.28](https://www.fda.gov/medical-devices/device-registration-and-listing/important-reminders-about-registration-and-listing).

For devices using the 510(k) pathway, FDA's overview explains the clearance determination through substantial equivalence. It also identifies circumstances where a 510(k) is not required. Do not assume every device needs one. [FDA: Premarket notification 510(k), Introduction and When a 510(k) is not required, checked 2026.10.28](https://www.fda.gov/medical-devices/premarket-submissions-selecting-and-preparing-correct-submission/premarket-notification-510k).

Those distinctions should determine how you organize the packet. Avoid a single folder called FDA approved that treats every attachment as interchangeable evidence.

## Give each attachment a question

Use a document index before the purchasing meeting. Record the filename, issuer, document type, issue date, retrieval date, and identifier exactly as supplied. Include a separate question column. That column should describe what the reviewer expects the attachment to establish.

Table: Suggested questions for separate document categories

| Category | Question for the reviewer | Information to request |
| --- | --- | --- |
| Establishment registration | Which establishment does this record identify? | Exact establishment name, identifier, and current record reference |
| Device listing | Which device and activity does this entry describe? | Listing reference, device name, and related establishment |
| Marketing authorization or exemption basis | What pathway is claimed for the quoted device? | Applicable identifier, source document, or supported exemption explanation |
| Product identity | How does the quoted item connect to those records? | Model, configuration, labeling, and supported name mapping |
| Commercial agreement | What will the seller supply and support? | Quote revision, item schedule, and written terms |

The table is a proposed review structure. It does not describe universal document requirements for every product or seller. Ask the regulatory reviewer which records apply to the particular transaction.

Keep unknown values visible. An empty identifier should read “not supplied,” with a named follow-up owner. Do not copy a different number into that field to make the worksheet look complete.

## Work through a fictional supplier packet

Imagine a fictional packet from Supplier B. It contains four attachments: a registration certificate from a private service, an undated listing screenshot, a product brochure, and a quote for Model R. None describes a real company or device.

The certificate names Example Device Works. The screenshot shows a device family without an exact model. The brochure says FDA registered. The quote names Model R with a Premium package suffix. There is no marketing authorization identifier or explanation of an exemption.

First, preserve the supplied files. Then write an index entry for each attachment without endorsing its title. Record the certificate's issuer as the private service named on it. Do not relabel that issuer as FDA.

For the listing screenshot, record the missing capture date and source URL. For the brochure, preserve the registration claim as seller wording. For the quote, extract the precise model and package label. Each action produces a concrete question instead of a general impression.

## Handle a registration certificate as an attachment

In this example, the private certificate cannot replace the missing authorization evidence. FDA's reminder distinguishes registration from authorization and says it does not issue these facility certificates. [FDA registration and listing reminders, checked 2026.10.28](https://www.fda.gov/medical-devices/device-registration-and-listing/important-reminders-about-registration-and-listing).

That observation does not establish that the fictional supplier has done anything unlawful. It establishes a limit on what the attachment answers. Keep the certificate in the file with an accurate description of its issuer and purpose.

Ask the supplier for the official record reference behind the registration claim. Request the exact establishment name and identifier used in that record. Ask your reviewer to explain any difference between the establishment, manufacturer, and seller names.

Keep that request separate from the marketing authorization question. Receiving a clearer registration record should close only the registration question. It should not silently change the status of the missing device evidence.

## Ask for a listing reference you can review

A screenshot needs enough context for another person to locate and interpret the entry. Request the official source, exact identifier, captured date, and complete relevant fields. Ask the supplier to explain how the entry connects to the quoted Model R configuration.

If the listing uses a different trade name, request a documented mapping. Preserve both names in the index. Avoid replacing one with the other before review. A corrected spreadsheet is less useful when it erases the discrepancy that prompted the request.

Do not assign a meaning to a missing field without evidence. Record “not visible in supplied screenshot” when that is all you know. That description differs from “does not exist” or “not required.” The latter conclusions need support your reviewer can inspect.

Use the same restraint when a search yields no result. Record the query, source, and date in the review notes. Ask for the controlling evidence rather than turning a failed search into a compliance finding.

## Request the claimed pathway without assuming a K number

For the fictional packet, ask which marketing pathway or exemption basis the supplier claims for Model R. Request the relevant identifier and official document where applicable. Ask the regulatory reviewer what additional evidence is needed to evaluate the answer.

If the response supplies a K number, send it to the [K-number, applicant, and model comparison guide](/articles/med-spa-device-quote-k-number-applicant-model-match). That process checks the relationship between the quoted item and the named document. A number pasted into an email is the beginning of the comparison.

If the response claims an exemption, request the specific basis and supporting source. Leave the conclusion with the reviewer qualified to evaluate it. Do not demand a fictional K number merely to complete a standard template.

If the supplier identifies another authorization pathway, preserve that answer in its own terms. Request the corresponding official record. The worksheet should accommodate the actual evidence rather than force every device into the same category.

## Send dated requests that name the missing evidence

A useful request states what you received, what remains unresolved, and which document would help. It also identifies the quote version being reviewed. The following is fictional draft text for an internal purchasing exercise:

> For the Model R quote dated October 28, 2026, we received a private registration certificate, listing screenshot, and brochure. Please provide the official registration and listing references, the exact model mapping, and the applicable authorization identifier or supported exemption basis. Please identify the date and issuer of each response document.

Choose a response date that fits your actual review schedule. Do not imply a statutory deadline through a purchasing request. Your team sets its own decision conditions with the appropriate advisers.

Give each open question an owner and a next review date. If the supplier replies with another brochure, attach it to the same question. Explain which missing identifier or document remains unresolved. Avoid restarting the conversation from an empty email thread.

## Route clinical, legal, maintenance, and purchasing questions separately

One reviewer should not receive an undifferentiated request to approve everything. Separate the document tasks from the decisions. Adapt the proposed roles below to the authority your practice has assigned.

Table: Suggested owners for the purchasing evidence checklist

| Question | Suggested owner | Record to keep |
| --- | --- | --- |
| What document and pathway support the stated regulatory claim? | Regulatory or compliance reviewer | Source references and dated interpretation |
| How do labeling and proposed use affect clinical review? | Medical director | Clinical review reference and unresolved questions |
| Which contract terms require legal review? | Legal adviser or authorized contract reviewer | Marked terms and written advice reference |
| What maintenance, consumable, and service documentation is missing? | Equipment or maintenance lead | Document requests and support commitments |
| Does the quote describe the intended item and configuration? | Purchasing lead | Quote comparison and supplier replies |
| What conditions must close before a decision? | Authorized decision maker | Decision conditions and responsible owners |

Use document references in the purchasing index rather than copying sensitive advice into a broadly shared folder. Keep patient information outside this exercise. The packet concerns equipment documents and business responsibilities.

The [public intelligence overview for med spas](/intelligence/med-spas-and-aesthetics) connects this review to an equipment evidence record. The [med spa business hub](/for/med-spas-and-aesthetics) provides the broader administrative context.

## Keep dates and status specific to the question

Use separate dates for the source document, your retrieval, the supplier's reply, and the reviewer's decision. A freshly received attachment can contain an older document. A recent email does not change the date of the evidence inside it.

Define completion for each question. For example, “official listing reference received” closes a request for that reference. It does not mean the clinical review passed. “Contract reviewed” does not establish device identity. Keep each decision beside the evidence it considered.

When the quote changes, compare the identifiers and configuration again. Reuse unaffected documents only after the responsible reviewer confirms their relevance. Mark superseded quote versions clearly while preserving the earlier decision history.

The [business memory guide](/articles/business-memory-for-ai-agents-guide) explains why source records, corrections, and decisions belong together. For purchasing, that means a later reviewer can see what the team knew at the time.

## Bring an organized packet to the scope discussion

Before asking for a document workflow, gather one redacted quote, the supplied evidence index, and your open questions. Identify the people who can resolve regulatory, clinical, contractual, and maintenance issues. Specify where the approved record should live.

Scale With Search can scope the document organization and review handoff from those materials. The resulting workflow should preserve evidence limits and reviewer decisions. The authorized people retain responsibility for the device and purchase conclusions.
