# Dental HPSA scores: prepare access questions without ranking practice profitability

A dental Health Professional Shortage Area (HPSA) score belongs beside a designation reference and a dated planning question. Before using it in a site discussion, establish which dental designation it describes. Then decide what your team needs to learn locally.

Keep the score visible as published evidence. Place proposed hours, staffing, payer participation, and partner arrangements in separate research columns. This article offers a question-sheet method for the owner and dental director to review together.

The [dental practice overview](/for/dental-practices) describes office preparation boundaries. The [dental intelligence overview](/intelligence/dental-practices) proposes a public-source planning record. This guide explains how a verified score can contribute to that record without becoming a practice-profitability ranking.

## Use the dental scoring guidance

The Health Resources and Services Administration (HRSA) gives dental HPSAs scores from 0 to 26. Population relative to providers contributes up to 10 points. The population share below 100 percent of the federal poverty level contributes up to 10. Water fluoridation status contributes up to 1. Travel time to the nearest care source outside the designation area contributes up to 5. [HRSA scoring guidance, Dental Health HPSA Scoring, reviewed September 2026; accessed October 28, 2026](https://bhw.hrsa.gov/workforce-shortage-areas/shortage-designation/scoring).

Use that list to prepare questions about the underlying access evidence. Do not reverse-engineer a local condition from the total alone. If scoring input values are unverified, label them unknown instead of assigning an explanation to the score.

For example, a preparer might write “high because of travel time” beside a total. Ask for the supporting record before accepting that explanation. When the source card contains only a total, the next task is to establish what can be verified about its inputs.

Keep your review inside the dental discipline. Do not borrow a scoring explanation from another health service because it uses the same acronym. Have the person preparing the sheet attach the dental-specific source section they used.

## Establish the designation context before comparing scores

The score comparison needs an identity check. Start with the designation reference, discipline, type, population, status, and relevant dates. Use [the designation-type site-study guide](/articles/dental-hpsa-geographic-population-facility-site-study) when those distinctions are unresolved.

HRSA distinguishes geographic, population, and facility HPSAs. A population designation describes a specified group within an area. [HRSA designation guidance, HPSA definitions, accessed October 28, 2026](https://bhw.hrsa.gov/workforce-shortage-areas/shortage-designation).

For a proposed comparison, put the two source cards side by side. Ask whether your question concerns the same kind of population and the same type of designation. If those differ, explain the difference before discussing the totals.

Keep the comparison question narrow. “Which published access signals should we investigate for this proposed service?” is useful. “Which location will earn more?” requires a separate commercial study that this worksheet does not provide.

Also inspect the dates. When one observation comes from an older saved record, do not describe the pair as a current comparison. Either reopen both references or label the mixed dates and explain why the older evidence remains in the file.

## Work through two fictional scores

Consider two synthetic dental designations, Study A and Study B. Assume the preparer has checked each source card for this exercise. A has a score of 18. B has a score of 14. Neither designation represents a real location or patient population.

The owner proposes visiting A first because its number is higher. Record that as a proposed research priority. Then ask what the visit should resolve. The score alone supplies no staffing roster, payer agreement, appointment schedule, partner commitment, or operating budget for the proposed practice.

Suppose the fictional team can document a possible evening staffing plan for B. A has no checked staffing proposal. Keep that distinction visible. It is a difference in the team's evidence, and it should not be disguised as a difference established by the federal score.

| Fictional study | Published-score assumption for the exercise | Separate local evidence state |
| --- | --- | --- |
| Study A | Dental score 18, identity checked | Staffing proposal unresolved; partner conversation pending |
| Study B | Dental score 14, identity checked | Evening staffing proposal available for internal review |

The next step could be a staffing review for A and a partner discussion for B. Both tasks can be useful. The example does not establish which service should proceed, and it does not convert four score points into money, appointments, or treatment need.

Now change the example. Suppose A describes a population designation while B describes a geographic designation. Add that difference to the first line of the comparison. Reframe the visit questions around the respective populations instead of treating the totals as interchangeable site ratings.

## Turn each input into a question your team can check

Use the published scoring inputs as headings for a discussion sheet. Under each heading, distinguish the source fact from the proposed follow-up. The following questions are editorial suggestions for research, not a substitute scoring formula.

For the population-to-provider input, ask what population the designation describes and what the underlying evidence says about providers. Then ask what separate information your practice needs about its proposed staffing model. Do not insert an office directory count as if it were a verified score component.

For the poverty input, preserve the designation context. Ask which administrative and access arrangements the practice should investigate for the proposed service. Have the responsible billing leader check any payer-participation question through the practice's normal process.

For fluoridation, record only what the cited evidence establishes. If no verified local input is available, leave a named follow-up. Keep questions about prevention or clinical service design with the dental director rather than drawing treatment conclusions from the total.

For travel time, retain the outside-designation care-source context in the research question. Ask which published reference supports the comparison. Separately plan any local observations your team needs about a candidate site, visit hours, or transport arrangements.

Each heading should end with an owner and a next action. “Need more research” is hard to review. “Practice manager will identify the current source and return the unresolved fields” states what the next meeting needs.

## Use a dated question sheet

Prepare the sheet before the planning meeting. The example below is dated October 28, 2026. Replace it with your preparation date, and keep later findings as dated additions rather than silently overwriting the original questions.

HRSA's dental layer metadata includes `HPSA_SCORE`, `DISCIPLINE_CLASS_DESC`, `HPSA_TYP_DESC`, `HPSA_STATUS_DESC`, and `HPSA_DESIGNATION_LAST_UPDATE_DT`. It also lists separate designation and component references. [HRSA dental layer metadata, fields, accessed October 28, 2026](https://gisportal.hrsa.gov/server/rest/services/HealthCareShortage/HealthCareShortage/MapServer/4?f=pjson).

Use a source card to retain those published labels. Keep the review sheet readable for the owner. Put field definitions and unresolved interpretation questions behind the sheet, where the dental director can inspect them.

| Question-sheet section | Entry to prepare | Review owner |
| --- | --- | --- |
| Published observation | Designation reference, dental discipline, type, status, score, and source dates | Research preparer |
| Comparison purpose | The service-access question this comparison supports | Owner |
| Evidence still missing | Unverified score-input detail and unresolved boundary questions | Dental director |
| Staffing | Proposed roles, hours, and internal confirmation needed | Practice manager |
| Payer participation | Current participation questions and approved verification path | Billing leader |
| Partner capacity | Proposed service discussion, responsible role, and unresolved commitments | Partnership owner |
| Next decision | Continue research, hold a question, or close it with a reason | Owner and dental director |

Use separate columns for published evidence, local findings, and assumptions. A proposed session belongs under assumptions until the responsible person checks it. If a finding changes, preserve what changed and who reviewed the replacement.

## Keep local capacity findings specific

Ask the manager to prepare one service scenario for review. It might involve a proposed evening session, a second candidate location, or a possible community partnership. Mark the scenario as provisional and define the question it is intended to answer.

For staffing, request a proposed coverage plan with an internal owner. Avoid filling the sheet with individual personnel details. The planning question is whether authorized leaders can establish suitable coverage for the proposed service.

For payer participation, ask the billing leader which arrangements need confirmation. Record the answer's source and date. Keep the published poverty-related scoring input in its own column, so nobody mistakes it for an office's participation status.

For hours, distinguish a suggestion from an approved schedule. Name who can confirm the proposed times and what operational questions remain. A draft calendar should carry its own review state even when the designation evidence has already been checked.

For partner capacity, prepare a bounded conversation question. Ask what the organization is able to discuss and who could confirm a commitment. Preserve a possible collaboration as a proposal until the responsible people establish its terms.

Bring these four reviews back to the same decision page. This lets the owner see which evidence is ready, which assumptions remain, and who must resolve them. It also keeps an attractive score from obscuring an unfinished local study.

## Handle a changed score as a research event

If a later source check shows a different score, retain both observations with their retrieval dates. Match the designation references before calling it a change. Check whether the status, type, or geography also differs between your saved records.

Prepare a change note that states what the team observed. Avoid assigning a cause that the retained evidence does not explain. “The saved observations differ” is a useful starting point when the source history still needs review.

Return to the decision the earlier score supported. Ask whether the new observation changes a question, creates a new check, or leaves the immediate next step intact. Have the responsible reviewer record that judgment in the planning file.

The [business memory guide](/articles/business-memory-for-ai-agents-guide) explains the value of preserving checked records. For this workflow, keep the source cards, question sheets, local findings, rejected assumptions, and dated decisions together. The next preparer should be able to reconstruct why the team chose a follow-up.

## Define the planning deliverable before requesting a build

Bring Scale With Search one territory, a bounded access question, and the people who will check the result. A proposed deliverable can contain the source cards, a score-comparison sheet, unresolved questions, and a dated review record.

Ask the written scope to name the source references, required fields, review roles, and handling of missing evidence. Keep operating feasibility, clinical decisions, financial analysis, and program eligibility with the responsible practice leaders and advisers.

The useful result is a question sheet your team can inspect against its sources. It should show what is known, what remains provisional, and what evidence the next decision requires.
