# Dental HPSA types: separate geographic, population, and facility designations in a site study

Before a dental site visit, write down whom the shortage designation describes. A county search is a way to find records. Your planning question must still follow each record's population, boundary, and designation type.

Start the worksheet with a complete sentence: “We are investigating whether this service model could address this access question for this designated population.” Leave the service model provisional. The worksheet should help the owner and dental director decide what to investigate before discussing a location commitment.

The [dental practice overview](/for/dental-practices) covers administrative work inside the office. This guide concerns public shortage records and site-study preparation. Its worked example, suggested questions, and decision labels are proposed methods for your team to adapt.

## Read the type before interpreting the county result

The Health Resources and Services Administration (HRSA) describes three Health Professional Shortage Area (HPSA) types. A geographic HPSA concerns a provider shortage affecting the population within a defined area. A population HPSA concerns a particular group within an area, such as people with low incomes. Facility designations concern institutions or facilities. HRSA also describes automatic facility designations and identifies state or county mental hospitals as a mental-health-only category. [HRSA designation guidance, HPSA definitions and facility categories, reviewed September 2026; accessed October 28, 2026](https://bhw.hrsa.gov/workforce-shortage-areas/shortage-designation).

Use the exact dental designation type in your worksheet. Avoid shortening every result to “shortage county.” That phrase drops the distinction the site study needs. Write the named population beside the area, even when the county name appears in both.

For a geographic record, prepare questions about the defined area and the services your proposed model could offer there. For a population record, ask how the proposed service would reach the specified group. For a facility record, identify the institutional context and who could explain its service arrangements.

Do not move a facility record into the geographic column because its location appears on a map. Do not assign a mental health designation to a dental study because both records mention the same county. Preserve the discipline as well as the type.

## Work through a fictional overlap

Imagine a fictional county called Example County. Your worksheet contains three synthetic records: Geographic A, Population B, and Facility C. These labels represent no real designation, place, practice, or person.

Geographic A covers a defined part of the county. Population B describes a specified low-income population in an area that partly overlaps A. Facility C concerns an institution within that overlap. For this exercise, assume each record has been verified separately and concerns dental care.

The owner proposes the sentence “Example County has three separate patient markets.” Reject that sentence. Your fictional records establish three designation contexts. They do not identify three distinct groups available to a practice, and the example explicitly includes overlap.

Instead, prepare three research questions. For A, ask which part of the defined area the site visit would examine. For B, ask what access arrangements the proposed service would need. For C, ask whether an appropriate institutional representative can explain a relevant partnership question.

| Synthetic record | Keep visible | First proposed question |
| --- | --- | --- |
| Geographic A | The defined boundary and dental discipline | Which part of this area would the proposed visit examine? |
| Population B | The named population and its boundary | What would make the service accessible to this specified group? |
| Facility C | The facility designation context | Who can explain the institution's relevant service arrangements? |

Do not total the three population entries. In this example, doing so would count overlapping people as separate markets. Keep one row per research question and explain the overlap in ordinary language. A shaded map can illustrate the question, but its caption must preserve the same distinctions.

## Keep a source card behind every worksheet row

HRSA labels the dental map layer “Dental Health HPSA - component polygons.” Its metadata exposes designation and component identifiers, discipline, type, status, county identifiers, score, and designation dates. Examples include `HPSA_SOURCE_ID`, `COMP_SOURCE_ID`, `DISCIPLINE_CLASS_DESC`, `HPSA_TYP_DESC`, and `HPSA_STATUS_DESC`. [HRSA dental layer metadata, layer name and fields, accessed October 28, 2026](https://gisportal.hrsa.gov/server/rest/services/HealthCareShortage/HealthCareShortage/MapServer/4?f=pjson).

Keep the source labels with their values. If your report displays friendlier headings, retain a small reference showing the original field behind each heading. Ask the preparer to identify an unfamiliar identifier before using it as a match key.

Use the following source card as a proposed minimum. “Not established” is an acceptable entry when the preparer cannot resolve a detail. It gives the reviewer a concrete task and prevents a blank cell from looking like a checked fact.

| Worksheet item | What the preparer should retain |
| --- | --- |
| Designation reference | Published identifier, name, and source link |
| Component reference | Component identifier, name, and relationship to the displayed area |
| Interpretation context | Dental discipline, exact type, and named population |
| Current observation | Designation status and any separately displayed component status |
| Geography | County identifier, boundary reference, and unresolved coverage questions |
| Evidence dates | Published designation dates, retrieval date, and worksheet review date |
| Local question | What the team still needs to establish before a decision |

Keep the dates separate. A retrieval date records when your team checked the source. Use a different cell for the date attached to the designation. Add a third date when someone verifies an operational fact locally.

## Review component rows before counting anything

Prepare two views for the owner. The first lists the designation references under review. The second shows the component references supporting each displayed area. Keep the connection visible when a designation appears beside more than one component.

In a fictional worksheet, A-01 and A-02 might be two components linked to Geographic A. Do not rename them “two shortages” because the export displays two rows. Ask the preparer to explain the relationship before choosing a label for the report.

Also distinguish an absent result from a completed coverage check. If the person preparing the worksheet cannot confirm that the requested area was fully inspected, label the research incomplete. Assign the missing check to a named role and retain the source request date.

The [dental intelligence overview](/intelligence/dental-practices) describes a proposed shortage change watch. For a site study, its useful handoff is a dated set of references and questions. Ask for that handoff before requesting a county ranking or a summary market number.

## Prepare the site visit around an access question

Choose one question that observation could help resolve. “Is this a good county?” is too broad. “What would an evening service session require at this candidate site?” gives the owner, manager, and dental director different facts to investigate.

Before the visit, have the owner state the candidate service model and the decision the study supports. Have the dental director review the description of the designated population. Ask the practice manager to identify the operational assumptions that remain untested.

Use a worksheet dated October 28, 2026, or replace that date with the day your team prepares it. Give each question an owner, evidence slot, and next review date. The following entries are prompts for investigation, not findings about a real place.

| Proposed visit prompt | Evidence to request or record | Reviewer |
| --- | --- | --- |
| How would someone reach the candidate entrance? | Observed route, visit date, and remaining transport questions | Practice manager |
| Which hours could the practice support? | Current staffing proposal and unresolved coverage | Operations owner |
| What access arrangements would the named population need? | Questions reviewed with a suitable community partner | Dental director |
| Which site assumptions need specialist review? | Open property or operating questions and responsible adviser | Owner |
| What should the team investigate next? | One bounded follow-up with a named owner | Owner and dental director |

Separate observation from interpretation in the notes. “We observed the entrance at this time” is an observation. “This entrance would serve the whole designation” is a broader conclusion that the visit alone has not established.

Record accessibility questions for the responsible specialists. Keep clinical service design with the dental director. The public designation should help frame the investigation without substituting for those decisions.

## Prepare a partner conversation without inventing capacity

Write the intended conversation outcome before contacting a potential partner. For this proposed workflow, the outcome might be permission to explore a defined service question. Do not describe a prospective organization as an established referral partner in the worksheet.

Prepare questions about the services the organization describes, the population it intends to serve, and the process for discussing a possible collaboration. Ask who can confirm hours, administrative requirements, and the limits of any proposed involvement.

Give each answer a source and date. A website description belongs in the published-evidence column. A representative's answer belongs in a separate conversation note with their organizational role. An unanswered email remains an unanswered question.

Use aggregated service questions. Do not request patient lists to make the worksheet seem more specific. Avoid recording individual treatment histories or household assumptions. The owner needs evidence about whether a service arrangement warrants further study.

After the conversation, have the responsible practice leader review the notes. Keep “discussed,” “offered for consideration,” and “agreed” as distinct states. Record what each party must verify before anyone describes an arrangement as available.

## End the study with a reviewable next step

Give the study a short decision page. State the designated population, the question examined, the evidence reviewed, and the uncertainties left open. Then record one of your team's chosen next-step labels, such as “investigate,” “hold,” or “close this question.”

For the fictional overlap, the team could choose to investigate an evening session for Population B while holding the property question for Geographic A. Facility C could remain a separate partner inquiry. Each decision needs its own reason and responsible person.

A decision to continue research should name the next evidence needed. A hold should name the unresolved assumption. A closed question should preserve the reason, so another planning cycle does not reopen it from an old map screenshot.

Store these reviewed notes with the source card and previous versions. The [business memory guide](/articles/business-memory-for-ai-agents-guide) explains how to keep a business-owned record for later preparation. Here, the useful memory is the checked distinction between the original signal and what the team learned.

For score interpretation, continue with [dental HPSA scores and access-planning questions](/articles/dental-hpsa-score-access-planning-questions). Keep commercial feasibility, permissions, clinical decisions, and program questions with the people responsible for them. This worksheet prepares their evidence.

Scale With Search can scope a source-backed planning record with defined references, worksheet fields, and review roles. Bring one proposed territory and one question your team needs to investigate.
