# Claude Remembers Incorrectly: Find the Source, Correct It, and Retest.

Claude carries an obsolete client preference into a new project and presents it as current policy. The preference appeared in an older chat, a project summary, and the account's remembered context. The client had reversed it in the approved brief.

The error may involve memory, chat search, project knowledge, instructions, or unsupported generation. Correcting one answer does not prove that the next project is clean.

Use a correction procedure in five parts. Identify the layer, and fix the authoritative record first. Then update the relevant Claude state, test project isolation, and leave a receipt.

## Classify memory, search, project, or generation

Anthropic's help center separates chat search from memory. As checked September 25, 2026, it says Anthropic moved users from a legacy memory experience to an improved one. The improved version shows memory under `Settings > Memory`. The legacy version shows it under `Settings > Capabilities`. Inspect the interface and record which experience the account has. Gather the failing prompt, response, account, project, model, date, and any cited source. Then inspect:

- memory settings and remembered entries;
- the source conversation found through chat search;
- project instructions;
- project knowledge files;
- profile or organization instructions;
- connected sources;
- the current owned record.

Classify the source as:

- `memory`, when Claude's managed memory contains the value;
- `chat_search`, when a prior conversation supplies it;
- `project_instruction`, when the rule is written in project guidance;
- `project_knowledge`, when an uploaded file contains it;
- `profile_instruction`, when account-wide guidance contains it;
- `generation`, when no inspected source supports it;
- `conflict`, when current sources disagree;
- `unresolved`, when the evidence cannot distinguish layers.

Do not tell Claude to forget everything. Broad deletion can remove valid working context and still leave the stale project file in place. The help center also lists a reset control that permanently deletes every memory on the account. Save that control for an account-wide decision by the data owner, not for one wrong fact.

## Correct the owned source before Claude

Open the authoritative client brief, policy, offer file, or decision record. Confirm the current value, owner, effective date, and superseded value.

If the owned source is wrong, correct it through the normal approval path. If it is already right, record that it passed inspection. Do not rewrite the timestamp to simulate a correction.

For project-specific facts, keep the identity in every receipt. A correct preference for Client Blue is still wrong if it appears in Client Red.

## Update Claude's stale state narrowly

For managed memory, use the current view and edit control to change or remove the specific entry. Anthropic's memory import and export guidance says you can open `Settings > Memory` and tell Claude what to change or remove. It also warns that memory import is experimental and still in active development. For chat search, preserve the old conversation as evidence when required. Promote the correction into the owned source. Do not depend on deleting history to establish the new rule.

For project knowledge, remove or supersede the stale copy after the project owner approves. Confirm that duplicate versions are not both active.

For project or profile instructions, remove changeable client facts. Point the instruction to the approved source and define the conflict behavior.

For unsupported generation, require citations for material claims and add a missing-source stop. A confident answer without a traceable source is not memory evidence.

For conflict, stop. The responsible record owners must resolve it before Claude prepares a material output.

## Inspect `claude-memory-correction-test.md`

Write a fixture that includes the wrong recall, current source, expected scope, and clean-chat result:

Keep the fixture synthetic when possible. If you need production evidence, use identifiers and controlled links, not sensitive client text in a broad test file.

## Retest inside and outside the project

Use the same harmless claim in each project but assign a different approved value. That makes cross-project leakage visible without exposing client information. Record the project identifier, active instructions, knowledge-file versions, memory experience, and clean-chat URL. Ask a second reviewer to compare the result with the fixture. If Claude returns the right value without the required source, mark the provenance check failed. Correct prose does not excuse a missing source.

Project isolation is part of the correction. Run the test list:

1. Open a clean chat inside the affected project.
2. Ask for the corrected field and its source.
3. Confirm that the current project file wins.
4. Ask with wording similar to the original failure.
5. Confirm that the stale value does not return.
6. Open a clean chat outside the project.
7. Confirm that the client-specific fact is unavailable unless explicitly supplied.
8. Open a different synthetic client project.
9. Confirm that no identity or preference crosses the boundary.
10. Remove the approved source from the test packet.
11. Confirm that Claude reports missing authority.
12. Export or copy the test receipt outside Claude.

Pass requires the correct value inside scope, no disclosure outside scope, source citation, and a stop when the source is absent.

## Check affected outputs and indexes

Search for the exact stale phrase, correction identifier, client ID, and names of summaries produced from the old record. Review each match in context.

Classify outputs as draft, externally sent, accepted record, cache, index, or archive. Drafts can be replaced. Sent messages may need a human correction. Accepted records need their owner. Caches and indexes need deterministic refresh. Archives should remain labeled as historical evidence.

Do not run an unbounded text replacement across clients. The same phrase can be valid elsewhere. Resolve identity and source before every change.

Close the incident only after the affected output owner accepts the correction and the clean test passes.

## Preserve an export and exit path

Anthropic provides account data export for individual accounts and organization-specific export controls for business plans. The individual export includes conversation and user data, but Anthropic says it cannot be imported into another personal Claude account through that mechanism. Keep the owned correction fixture outside the product. Test it in another model or local search tool. If the correction works only because Claude still has the original conversation, the business state remains trapped.

## Approval and stopping boundary

The correction workflow may inspect allowed Claude settings, search permitted chats, prepare a targeted memory change, update a synthetic fixture, and test project isolation. It stops before deleting material conversation evidence, changing a client's approved brief, exporting protected organization data, modifying shared project instructions, or sending a correction externally.

The record owner approves the current value. The project owner approves project changes. The data owner approves export and deletion. The client or account owner approves external corrections. If identity, source, scope, or approver is unresolved, preserve evidence and stop.
