Confirm the version, intended discipline, reading order, fields, and return instructions. Keep blank public forms separate from completed patient records. Start with a list of every form linked from the chiropractic and physical therapy pages. Include old download links that staff still send to patients. ## Give each download a current owner Have the front-desk or clinical owner confirm which form belongs to each service and location. Keep clinical wording and consent language with the responsible clinic reviewer. Include headings, instructions, fields, checkboxes, and any links. A good-looking preview is not the acceptance record for those checks. Request findings that identify the document version, task, barrier, and repair. Retest the repaired file rather than relying only on the editor's export settings. ## Give fields clear labels and instructions W3C's form guidance recommends labels that identify controls and explain their purpose. Check whether the person using a keyboard can identify and reach each required control. Keep instructions about required and optional information consistent with the approved clinic process. Have the clinical or privacy owner resolve unnecessary requests rather than asking the designer to choose them. ## Keep the website instructions outside the download too Explain the document's purpose and format in nearby page text. Tell visitors what to do if they cannot use it. Have staff verify the assistance contact and the alternatives they can support. Do not invent an email-upload route for completed forms.
