# Medicare service counts and beneficiary counts: prepare a clinic service-mix worksheet

Before comparing local service activity, write the unit beside every number. A service count, a distinct-beneficiary count, and a count of provider-service combinations answer different questions. A worksheet that labels all three as patients can misdirect a clinic's capacity discussion.

Start with one reporting year, one documented service selection, and separate columns for each measure. Use the public extract to prepare questions for the practice owner. Confirm current capacity through independent business checks before discussing staffing, space, or equipment.

The [clinic intelligence overview](/intelligence/chiropractic-and-physical-therapy-clinics) introduces historical service-mix research. This worksheet develops the measurement step. It does not establish clinical need, referral relationships, or the number of people available to become patients.

## Freeze the release before choosing columns

The CMS catalog inspected on October 28, 2026 identifies a provider-and-service release covering calendar year 2024. Its metadata lists May 21, 2026 as the modification date. Those dates describe different things: the service period and the published resource's modification. Keep both in the workbook. [CMS official catalog, accessed 2026.10.28](https://data.cms.gov/data.json).

Save the selected dataset title, reporting interval, dictionary link, methodology edition, retrieval date, and filter notes. A person reviewing the worksheet later should know which evidence produced it. A page's current appearance alone cannot document an earlier extract.

Use the dictionary attached to that release. Do not transfer field definitions from a hospital dataset or a provider summary. Similar product names can conceal different units. The relevant document here is the [2024 provider-and-service dictionary, accessed 2026.10.28](https://data.cms.gov/data-api/v1/dataset/335e5f35-eca6-482d-87b3-f99883e213e3/dictionary).

Write a narrow planning question before collecting numbers. For example: “Which approved service categories deserve a discussion of our current appointment capacity?” That question supports a reviewable comparison. “How many local patients can we acquire?” asks this source to establish facts it does not contain.

## Identify what one row represents

CMS aggregates the detailed file by rendering provider NPI, HCPCS code, and place of service. One provider can therefore contribute several rows. Changing the procedure or setting changes the combination. [CMS methodology, March 2025, section 4.1](https://data.cms.gov/sites/default/files/2025-04/MUP_PHY_RY25_202350312_Methodology_508.pdf).

For the clinic worksheet, retain that combination until the reviewer approves an aggregation. Give each retained row a local reference such as “example row A.” Public presentations can show aggregate findings without reproducing practitioner identities.

The dictionary defines `Place_Of_Srvc` as facility (`F`) or non-facility (`O`). Non-facility generally means an office setting, but includes other entities. Avoid renaming every `O` observation “clinic office.” [CMS 2024 dictionary, Place_Of_Srvc](https://data.cms.gov/data-api/v1/dataset/335e5f35-eca6-482d-87b3-f99883e213e3/dictionary).

Keep a row-count column labeled “published provider-code-setting combinations.” It describes the selected extract. It does not count physical offices, available appointments, or independent businesses. One organization label should not erase the underlying combinations used to calculate a total.

## Give each utilization measure its own definition

The 2024 dictionary supplies three measures that deserve separate worksheet columns. Preserve their exact field names in a reference tab even when the presentation uses shorter labels. [CMS 2024 dictionary, utilization fields](https://data.cms.gov/data-api/v1/dataset/335e5f35-eca6-482d-87b3-f99883e213e3/dictionary).

| Field | Worksheet label | Reading rule |
| --- | --- | --- |
| `Tot_Srvcs` | Reported services | The counting metric can vary by service |
| `Tot_Benes` | Distinct beneficiaries within this combination | Uniqueness applies to the provider, code, and setting combination |
| `Tot_Bene_Day_Srvcs` | Distinct beneficiary/day services | Keep separate from both services and annual distinct beneficiaries |

The dictionary explains that the beneficiary/day measure removes repeated same-type services for a beneficiary on a single day. It does not convert the detailed file into a count of unique people across providers. Avoid labeling that measure “appointments” without separate evidence that supports that interpretation.

The warning about service metrics matters before combining codes. Ask the billing reviewer whether the chosen codes have compatible counting units. Until that review is complete, display separate code-level figures. A convenient spreadsheet sum does not make unlike quantities comparable.

## Work through a synthetic comparison

The following numbers are invented for instruction. Provider labels and service labels are placeholders, not practitioner identities or procedure descriptions. Assume all rows belong to the same selected reporting year and setting.

| Synthetic combination | Reported services | Distinct beneficiaries | Beneficiary/day services |
| --- | ---: | ---: | ---: |
| Provider A, service X, non-facility | 240 | 40 | 120 |
| Provider A, service Y, non-facility | 90 | 30 | 60 |
| Provider B, service X, non-facility | 180 | 45 | 90 |

There are three published combinations in this example. Within service X, the reported-service sum is 420. Across all three rows, the arithmetic service sum is 510. Before presenting the latter as a useful total, confirm that combining X and Y is meaningful.

The beneficiary column adds arithmetically to 115. That result is a sum of row-level counts. It does not establish 115 distinct people. A person could appear in both service rows for Provider A, in rows for both providers, or in several combinations.

For a synthetic overlap demonstration, suppose Provider A's two service groups share twenty beneficiaries. Their union would then contain fifty people: forty plus thirty minus twenty. The public row counts alone do not supply that overlap. The invented overlap explains why addition fails; it supplies no estimate for a real territory.

Do not solve this by assigning an assumed duplication percentage. Keep “distinct people across selected rows” unresolved. The spreadsheet can carry an explanatory note instead of a fabricated total. Your planning question can still proceed using properly labeled observed activity.

## Make ratios answer a narrow question

In the synthetic first row, 240 services divided by forty beneficiaries equals six reported services per beneficiary for that combination. The label must preserve the year, code, and setting. It is not six appointments per person across the practice.

A higher ratio in another row needs interpretation. Ask whether service units match, whether settings differ, and whether the comparison addresses the owner's question. Do not assign a clinical explanation from the ratio alone.

Avoid averaging row ratios without choosing a defensible weighting rule. For service X in this example, adding its service counts and beneficiary counts yields 420 divided by 85. The denominator still contains potentially overlapping people. Calling that result “services per unique local patient” would introduce an unsupported claim.

A useful worksheet can omit this combined ratio entirely. Show the two code-level observations and ask the reviewer what comparison would help. An empty analysis cell is preferable to a polished metric whose denominator changes meaning halfway through the calculation.

## Keep population and geography beside the result

The March 2025 methodology describes final-action physician/supplier Part B non-institutional claims for the Medicare fee-for-service population, excluding DMEPOS contractor claims. It does not describe a census of all clinic activity. Its limitations exclude Medicare Advantage coverage and other payer populations. [CMS methodology, sections 1, 3, and 5](https://data.cms.gov/sites/default/files/2025-04/MUP_PHY_RY25_202350312_Methodology_508.pdf).

Use a visible scope label such as “historical published Medicare Part B fee-for-service activity in selected combinations.” Do not shorten that to “local demand.” A private-pay service line could be important to a clinic while remaining outside this source's population.

The dictionary describes `Rndrng_Prvdr_Zip5` as the provider ZIP reported in NPPES. It is not a beneficiary residence field. A provider-ZIP selection therefore cannot establish where the represented beneficiaries live. [CMS 2024 dictionary, Rndrng_Prvdr_Zip5](https://data.cms.gov/data-api/v1/dataset/335e5f35-eca6-482d-87b3-f99883e213e3/dictionary).

The methodology places the NPPES demographic snapshot at the end of the year following the reporting year. Record that timing separately from the service period. Do not present the address as a verified location for every claim. [CMS methodology, section 2](https://data.cms.gov/sites/default/files/2025-04/MUP_PHY_RY25_202350312_Methodology_508.pdf).

Call the resulting geography a provider-location selection. If the owner needs a patient catchment study, record that as a separate research question. Do not infer travel distance, neighborhood residence, or referral flow from provider-location fields.

## Preserve omitted activity as a limitation

CMS excludes detailed aggregated records derived from ten or fewer beneficiaries. Consequently, a sum of published service rows can omit activity within the covered population. [CMS methodology, sections 3 and 5](https://data.cms.gov/sites/default/files/2025-04/MUP_PHY_RY25_202350312_Methodology_508.pdf).

Do not fill missing combinations with zero before adding the observed rows. Give the total a coverage note and identify it as a published-row total. The companion guide explains [how to preserve missing service rows in a planning brief](/articles/clinic-medicare-suppressed-service-rows-unknown).

Use the same rule when comparing years. A row appearing in one extract and absent in another does not establish that a service started or stopped. Check source definitions, selection rules, and publication coverage before treating the difference as a business event.

## Bring a review worksheet to the capacity meeting

Separate source interpretation from the owner's current operational evidence. The following worksheet is a suggested meeting aid. It does not describe a rule imposed by CMS.

| Review item | Entry to prepare | Reviewer question |
| --- | --- | --- |
| Source edition | Dataset, year, dictionary, access date | Can another reader reproduce the selection? |
| Service grouping | Approved code list and rationale | Are the units compatible? |
| Geography | Provider-location filter | Does this match the planning territory? |
| Published counts | Services and beneficiaries separately | Which totals preserve their stated meaning? |
| Coverage | Missing-row and payer limitations | What remains unknown? |
| Current capacity | Clinic-approved aggregate information | What can the clinic offer now? |
| Follow-up | Owner, question, due date | Which evidence could change the decision? |

Ask the clinic manager for current scheduling constraints in aggregate form. Ask the clinical lead which proposed service comparisons make sense. Ask the billing reviewer to confirm the selected definitions. Keep individual patient information outside this worksheet.

The meeting's output can be a short list of questions rather than an expansion recommendation. For example: confirm whether a proposed service belongs in the comparison, check present appointment availability, and identify the missing all-payer evidence. Each question should have an owner and an evidence requirement.

## Keep the interpretation with the worksheet

Store the approved definitions, rejected calculations, source versions, and reviewer corrections beside the final table. A later analyst should see why the beneficiary column has no territory-wide total. The [business memory guide](/articles/business-memory-for-ai-agents-guide) explains how to preserve reusable decisions and their supporting records.
