State which records it may read, which professional checks its output, and which actions remain unavailable. Put a cost limit beside the permission list. Use this guide to prepare a review sheet for the clinic's privacy, clinical, billing, and office owners. The proposed controls focus on administrative preparation. This guide covers the rules needed before patient information enters one of them. ## Establish HIPAA coverage from the actual operation HHS says the HIPAA Rules apply to covered entities and business associates. Its provider definition includes the condition of transmitting information electronically in connection with an HHS-standard transaction. Do not infer it from a wellness label, payment preference, or shared reception desk. Keep other applicable professional and privacy requirements in the review even where HIPAA status differs. Name the person responsible for each discipline. A shared company policy can define office roles, but the reviewer for a clinical statement should match the work. Keep chiropractic, physical therapy, scheduling, and billing authority explicit. Choose a first function whose inputs and outputs the reviewers can describe. If its purpose requires new clinical reasoning, return it for a separate scope. Start administrative preparation with the information needed to assemble a packet. ## Review the full data path HHS says a cloud provider handling electronic protected health information for a covered entity is a business associate, including when it holds encrypted information. An appropriate agreement and risk analysis remain necessary. Ask the maintainer to map input collection, storage, processing, output, logs, support access, and backup. Identify the actual service and account configuration. Do not stop the review at the chat window or upload button. Have the privacy owner approve the permitted purpose, recipients, retention, and incident route. Keep that decision with the function instructions. A general security statement from a vendor should not replace review of the configured data path. Until approval is complete, use invented patients and documents. Mark synthetic fixtures clearly. Keep real appointments, referrals, histories, and claim documents out of a general development folder. ## Classify records for a specific function Create an allowlist for each job. A scheduling draft should not read the full clinical history merely because the connector makes it available. Have the clinic approve exceptions and the actual access roles. Keep the effective permissions narrow enough that the function cannot retrieve prohibited records in the first place. Use separate controls for each patient and discipline. Do not ask the model to retrieve broadly and remove inappropriate detail only in its final answer. Test source access as well as the finished draft. ## Treat de-identification as a reviewed process HHS describes Expert Determination and Safe Harbor as methods for satisfying HIPAA's de-identification standard. Its guidance includes identifiers in free text, as well as structured fields. Do not declare a narrative de-identified because staff removed a name. Ask the privacy owner to approve the method and resulting dataset. Use fully invented examples when the function does not need real patient history. Keep internal patient references within the approved workspace. A reference useful for matching records does not, by itself, establish de-identification. Give cost reports and general test reports only the identifiers their own approved purpose requires. Ask the builder to inspect failed-run files and logs during acceptance. A blocked upload should not leave its full content in a general error report. Include those destinations in the data-flow review. ## Assign approval by the proposed action Have the front desk approve administrative wording and scheduling facts. Have the relevant clinician approve clinical statements. Have the billing owner approve balance and payer language. Keep uncertain routing visible until the clinic resolves it. For a referral packet, distinguish received documents, recorded payer information, clinician review, and confirmed scheduling. Require the controlling record for each. Do not use one “ready” label to imply that all decisions are complete. For a scheduling message, bind approval to the patient, recipient, channel, exact text, and current options. If a source or recipient changes, return the message for another check. Keep booking and moving appointments unavailable in the first draft-only function. For a billing handoff, keep balance changes, claim submission, code changes, and fee waivers outside the function. Require the responsible person to resolve the authoritative record before preparing a corrected reply. ## Keep patient communications out of general publishing Give the AI function no authority to convert a patient question into a public FAQ, testimonial, or case example. Require a separately reviewed publication process for any patient-derived material. Use approved general office facts for public explanations. Keep that inventory and the internal AI data path separately documented, then have the privacy owner review any connection between them. Record the exact approved release version. Save the recipient and channel check with it. An internal draft can be complete while external release remains blocked. Make those states visible to the staff member responsible for the next step. ## Keep absence cover within the approved roles Name a deputy for each administrative reviewer before handoff. Have the clinical lead define cover for questions that need professional judgment. The function should never choose a substitute based on who happens to be available in the staff directory. Require an absent-owner fixture. Remove the assigned reviewer from a synthetic case and leave no approved deputy. The expected result is a held packet with a named office escalation route. Keep the function unable to release the output while that assignment remains unresolved. ## Control cost per function and provider Give referral summaries, scheduling drafts, and billing handoffs separate monthly allowances. Set a maximum cost per run, bounded document volume, and retry limit. Name the alert recipient and person who may approve more spend. Require a budget check before the next paid call. Make exhausted allowance stop the run. If the current service fails, do not send patient material to an unreviewed replacement to finish the job. Name approved models in internal settings and receipts. Require a replacement to pass the same privacy, source, approval, and output checks. Every run should record its function, source versions, status, model, and cost without unnecessary patient detail. ## Accept the rules through controlled failures Test a forbidden clinical attachment, wrong patient, conflicting authorization, wrong discipline, changed recipient, attempted send, and exhausted allowance. Require the expected stop in each case. Include a complete synthetic packet that staff can check from the sources. Have an authorized staff member run the instructions and locate the receipt. Preserve the accepted policy, fixtures, and corrections under clinic control. Reopen review when the data path, provider, permissions, or purpose changes.
