# AI-native medical billing: the first functions to build

Start an AI-native medical billing company with the administrative work around a claim. Choose a function that prepares a file checklist, highlights a changed client instruction, or drafts an evidence-based status report. Keep coding, claim submission, clinical documentation, and payment decisions with authorized people.

A first function needs an output someone can accept. Define the source set, director, checker, stop conditions, and spending limit before choosing a model. Keep the existing billing system authoritative for its records and actions.

The [medical billing hub](/for/medical-billing) describes the office records. This plan turns them into a sequence for selecting and handing over a preparation function.

## Study the office handoff before the claim logic

Ask the account lead to walk through a permitted batch from receipt to its next staff action. Record where staff look up required files, accepted client instructions, work status, and unresolved questions.

Ask which records the team copies into another note and why. Identify the decision supported by each copy. A duplicate progress sheet may be removable. A fixed completeness check may need no language model. A readable account of contradictory instructions may justify drafting assistance.

Keep the study separate from medical necessity, code selection, and reimbursement interpretation. The first administrative function should have a boundary the account lead can inspect without asking AI to resolve clinical or payer judgment.

## Establish the information-handling boundary

HHS lists billing and claims processing among activities that can make an organization a business associate when they involve protected health information. [HHS business-associate guidance, checked 2026.10.26](https://www.hhs.gov/hipaa/for-professionals/privacy/guidance/business-associates/index.html).

Have the privacy owner approve the actual source and processing arrangement. Keep patient information outside a development trial that can use synthetic records. A business-level checklist can often be designed using invented file references and administrative states.

HHS generally requires minimum-necessary limits for covered uses, disclosures, and requests, with specified exceptions. [HHS minimum-necessary guidance, checked 2026.10.26](https://www.hhs.gov/hipaa/for-professionals/privacy/guidance/minimum-necessary-requirement/index.html).

Have the privacy owner identify which fields the chosen task needs and which rules apply. Do not let a convenient full export become the default input. Define the permitted fields and record that decision before implementation.

## Sort recurring work into four decisions

Review the task list with account operations and privacy:

| Decision | Billing-office task to examine |
| --- | --- |
| Eliminate | A duplicate client-status sheet with no separate operational purpose |
| Automate | Comparing a required-file list with a received-file index |
| Augment | Preparing contradictory instruction changes for account-lead review |
| Outsource | A defined specialist review with approved data access |

Keep the sorting record with the study. It should explain why a task remains, changes, or stops. Removing a duplicate task is a complete operating decision even when no AI function follows it.

For specialist work, name the question, source authority, recipient, and permission. The specialist's assignment should not automatically grant access to every provider or patient account.

## First function: prepare a file-completeness packet

Have the operations lead direct this function. Supply the approved required-file checklist, received-file index, and selected administrative batch identifiers. The assigned account lead checks the output.

Use fixed comparisons to identify missing references, duplicate entries, and unreadable-file states. Let the drafting step organize the findings into received, missing, awaiting review, and unresolved. Keep receipt distinct from acceptance.

Each item should point to the authoritative source. A file with a familiar name should not become proof that it contains the required material. Staff check the contents through the approved system.

Use a synthetic case where the index says a file arrived but its recorded batch reference differs. The expected packet flags the mismatch and names the account lead. It should not close the item or declare the batch ready for submission.

Accept this function when a checker can trace every completeness statement. Require an explicit no-work receipt when no batches qualify. An empty output should not conceal a failed run.

## Second function: prepare an instruction-change handoff

Have the account lead direct this function using the current approved client instructions, incoming change request, and decision record. A designated operations reviewer checks the comparison. The authorized client contact resolves acceptance where needed.

Require the output to separate the current rule, requested change, affected work, effective-date question, and decision owner. Preserve conflicting instructions without choosing which one wins.

Use a synthetic case where a new message requests a different file format but supplies no accepted effective date. The function should identify the request and the affected batches. It should not rewrite the controlling client instructions or apply the change retrospectively.

Keep payer-policy research outside this first function. The existing [medical billing memory guide](/articles/ai-for-medical-billing) covers payer-rule and denial context. This handoff addresses the office's accepted instructions and unresolved administrative changes.

## Third function: draft a client status report

Build this after the team agrees on work states. Have the account lead direct the report using the work log, open-item list, approved measurement definitions, and selected reporting period. The delivery manager checks the evidence. The account owner approves the outgoing message.

Keep completed office steps, pending client answers, unresolved files, and next approved actions in separate sections. Do not turn queued work into submitted claims or infer payment from a staff completion note.

If the report includes a measurement, require its definition, period, and source. Show a missing value as unresolved. Do not estimate a recovery result or insert a familiar percentage to complete the format.

Test a changed reporting period and an unavailable source. The draft should preserve the limit and identify the owner needed next. A polished narrative is not acceptance evidence when its work states cannot be checked.

## Keep client records and corrections separate

Require one client identifier and approved source set per run. Keep shared methods free of patient details, account-specific instructions, and retained client examples. Test the boundary using two synthetic accounts with distinct invented markers.

Store the specification, source map, allowed fields, role list, corrections, and receipts in business-owned records. Current billing and clinical systems retain authority. Define the access or exports the function needs rather than replacing those systems in the first scope.

Date accepted instruction changes and identify the tasks affected. Recheck a synthetic example after a correction. The next run should follow the accepted source without another employee explaining the history.

Keep the receipt limited to its checking purpose. Record function, client identifier, source versions, output location, model identifier, cost state, and completion state. Protect it under the approved information-handling plan.

## Put spend controls around the preparation work

Give completeness packets, instruction handoffs, and reports separate monthly allowances. Name a budget owner and backup. Cap batches, source pages, paid requests, retries, and output length per run.

Set an alert threshold for each function. Notify its budget owner before the monthly allowance is exhausted.

Check remaining allowance before another paid request. Reserve for pending work and stop when the next request would exceed the approved ceiling. Keep an unresolved charge marked pending until it is reconciled.

Maintain a manual route for the administrative task. A model outage or budget stop should produce a clear work status and responsible owner. It should not silently drop an open file question.

Keep model choices in private configuration. Rerun the same privacy, account-boundary, missing-file, and instruction-conflict examples before accepting a replacement. The business retains its records and acceptance method through that change.

## Hand over the first function

Use 10/80/10 as the working method: a person directs, the system prepares the middle, and a person checks. Treat the proportions as responsibility design, without claiming measured savings or a claim outcome.

Ask a backup account lead to run the function from its written instructions. Include a wrong-client reference, missing source, conflicting instruction, unknown cost, and ordinary completed packet. Save expected and observed behavior.

Bring the administrative job to [AI systems](/ai-systems). Send Scale With Search its permitted sources, named reviewers, and acceptance checks through [the written scope form](/work).
