# AI-native dental practices: the first functions to build

Start an AI-native dental practice with one administrative function that staff can direct and check. Choose a repeated handoff with a clear source, a defined result, and an owner. Keep treatment decisions with the dentist.

For example, prepare tomorrow's list of unresolved appointment paperwork. The system can assemble approved administrative facts and flag missing records. The front office checks the list before anyone contacts a patient or changes an appointment.

The [dental practice overview](/for/dental-practices) describes individual preparation jobs. This article addresses the earlier management decision: which function should your office build first, and what must exist around it?

The proposed functions below are starting designs. They do not certify a practice's privacy arrangements, replace its clinical system, or authorize clinical use.

## Study the handoffs before selecting a tool

Ask the front desk, treatment coordinator, billing owner, and dentist to describe one ordinary working day. Trace a request from arrival to its recorded resolution. Write down every place where someone copies, checks, or explains the same fact.

Include intake, appointment requests, treatment estimates, benefit questions, patient follow-up, and daily office reporting. For each task, record its trigger, source, result, and unresolved decisions. Distinguish administrative preparation from clinical interpretation.

Ask staff to show the actual controlling record. A saved message and an approved estimate can contain different information. Name which one controls each field. If nobody can identify the current version, fix that record before building a drafting function.

Use a small sample approved for the study. Begin development with fictional records. Patient information needs a separate, documented access decision before it enters any service.

## Sort the work into four decisions

Use this worksheet to choose what deserves a function. These are proposed choices for the office to review.

| Decision | Dental-office example | Owner's next step |
| --- | --- | --- |
| Eliminate | A second status sheet that repeats the current scheduling record | Identify who reads it before retiring it |
| Automate | Check whether required administrative fields exist in an approved export | Define exact fields and missing-value behavior |
| Augment | Prepare an estimate follow-up or unresolved-question brief | Assign the staff reviewer |
| Outsource | A task requiring outside expertise the practice has chosen to obtain | Define access, deliverables, and accountability |

Do not use a language model for an exact presence check or a fixed arithmetic rule. Reserve model work for preparing language or organizing varied notes within an approved boundary.

Remove duplicate work deliberately. Keep required clinical, billing, and privacy records intact. The study should identify a replacement source before proposing retirement of any working document.

## First function: prepare the administrative morning brief

Give the office manager direction of this function. The front-desk lead checks its result. In a small practice, one person may hold both roles, but the checking step remains explicit.

The approved inputs might include the upcoming appointment export, the administrative document checklist, and recorded requests awaiting an office response. Limit the function to that set. Do not connect the entire chart archive because it is available.

The output is an internal brief grouped by office action: missing paperwork, unresolved location questions, appointment requests awaiting confirmation, and items needing a named staff decision. Each entry points to its source and states what remains unknown.

Keep clinical concerns out of an administrative prioritization score. Route a patient's reported concern under the practice's approved process without classifying its severity. The dentist or designated clinical staff handles that decision.

Test a missing document, a canceled appointment, two patients with similar names, and a changed office location. The brief should preserve uncertainty and avoid joining unrelated records. A completed brief does not confirm or move an appointment.

## Second function: stage treatment-estimate follow-ups

Let the treatment coordinator direct this function and check its administrative wording. Require a dentist to review any clinical statement that falls outside already approved text. Billing checks benefit or balance language when it appears.

Supply the current approved estimate, the recorded patient question, contact restrictions, and the follow-up template. Name the estimate version in the result. Keep requested treatment separate from treatment the dentist has approved.

The function prepares a draft and an exception list. It can copy recorded procedure descriptions and estimate terms. It should identify missing information without inventing a fee, interpreting a benefit plan, or predicting payment.

For a useful test, give it an earlier estimate beside a corrected version. It must use the designated current record or stop if authority is unresolved. Add a patient request to stop contact. The expected result is a hold, with no follow-up draft offered for sending.

Build this function only if staff can maintain its source records. Automating a follow-up around an uncertain estimate creates another document for the coordinator to reconcile.

## Third function: assemble the office exception report

Assign direction to the practice manager. Have the relevant scheduling, billing, or treatment owner check each section. This function should summarize the work already prepared, rather than reread every patient record.

Use run results and staff dispositions as inputs. Separate a draft awaiting review from a message staff sent. Separate a request received from an appointment confirmed. Record the person responsible for the next step.

The report can show missing-source problems, repeated corrections, and drafts waiting for approval. Calculate counts deterministically from recorded states. Ask the model to explain only the exceptions supported by those states.

Do not treat a larger draft count as a business result. Review whether staff can locate the source, understand the uncertainty, and make the next decision. Keep the reporting function internal until the office has accepted its handling of identifiers and access.

Start here only if fragmented status reporting is the primary burden. Otherwise, build the underlying preparation function first and add reporting after its states are dependable.

## Establish the patient-data boundary first

HHS describes when providers are HIPAA covered entities and how the Privacy Rule protects identifiable health information. Determine your practice's status before choosing the data path. [HHS Privacy Rule summary, checked 2026.10.26](https://www.hhs.gov/hipaa/for-professionals/privacy/laws-regulations/index.html).

For a covered practice, a cloud service processing electronic protected health information on its behalf can be a business associate. HHS requires the applicable agreement and safeguards. [HHS cloud guidance, checked 2026.10.26](https://www.hhs.gov/hipaa/for-professionals/special-topics/health-information-technology/cloud-computing/index.html).

Have the privacy owner approve the exact service, account, data classes, and use. Keep patient details out of an unapproved AI account. A proposed administrative use still needs that review.

Use the separate [dental AI rules](/articles/dental-practice-ai-rules-data-approvals-spend) to specify approvals, restricted records, and cost controls before implementation.

## Give every function a cost boundary and a handoff test

Write a monthly model-spend limit for each function. Name the practice manager or other budget owner who receives an alert. Add a per-run ceiling, bounded retries, and a stop before the next request exceeds available budget.

Require a receipt stating the function, source versions, result, exact model identifier, and cost. If billing information arrives later, label the initial cost as estimated. Reconcile it with the provider's usage record. Failed runs belong in the spend total.

Use the 10/80/10 method as a division of responsibility: staff set direction, the system prepares the middle, and staff check the result. The proportions describe the method. They are not a measured time-saving claim.

At handoff, have an office employee run normal, missing-source, wrong-patient, and budget-stop examples. Require the employee to explain the block and correct the controlling record. Preserve the correction for the next run.

Scale With Search studies the business, sorts the recurring work, and builds one working function at a time. Your practice keeps the records, rules, receipts, and training. Review [AI systems](/ai-systems), then [send your brief](/work#send-your-brief) with the office handoff you want to address first.
