# Keep your clinical reference in one file so AI help matches how you practice.

It is Tuesday morning, and you are three appointments in. A Labrador with chronic ear infections is back for the fourth time this year. You remember a note about a grain-free diet trial, but you are already 15 minutes behind. You ask the owner the same questions you asked last month, because the details are not fresh.

That is the context problem in veterinary medicine. You see many patients a week. Each animal has an owner with preferences and concerns, and a history you are expected to remember. Each case has treatment notes, medication trials, and behavior patterns that matter for diagnosis.

Standard AI tools do not help much here. A fresh ChatGPT chat does not carry last month's case forward. So you use AI for a quick interaction check or a dose calculation, then go back to memory and the chart for everything else.

## Keep the chart as the record

The practice management system holds the medical record: patient history, allergies, prescriptions, and client communication notes. It stays the record. An AI setup should read from it, not compete with it.

The chart does not hold your own clinical reference. That reference includes the protocols you refined and the species notes you rarely need. It also includes the technique details that work in your hands and the local knowledge you gathered over years. That knowledge fits in a file that Claude reads before it helps you.

Patient and client data need care. Many states have laws on the confidentiality of veterinary records, and your practice may have an AI policy. The files stay on your computer, but Claude Code sends the text it reads to Anthropic's servers during a session. Retention and training terms depend on your plan.

## Protocols you reuse

Some protocols you use often: ear cleaning for bulldogs with stenotic canals, sedation for fractious cats, and your step-by-step approach to diabetes regulation in dogs. Write them in the reference file once. When a case calls for one, Claude drafts the plan or the client instructions from your version.

## Drug interactions: your notes beside the formulary

Every veterinarian carries a mental list of dangerous combinations: NSAIDs with corticosteroids, some antibiotics with seizure medications, and anesthetic agents that do not mix.

You add to that list all the time. A dog reacts badly to a common drug. A new study reports an interaction. A specialist mentions a contraindication during a consult.

Record those additions in the reference file with the date and the source. Keep your formulary, such as Plumb's Veterinary Drug Handbook, as the authority for doses and interactions. Tell Claude in `CLAUDE.md` to cite your file or the formulary for any drug statement, and to say "no entry" otherwise. A dose from a model's general knowledge does not belong in a treatment plan.

A patient-level allergy, such as a golden retriever's reaction to cephalosporins, belongs in the chart. When you consider antibiotics for that dog next week, the chart alert is what protects the patient.

## Species-specific notes for rare cases

Exotic medicine needs different knowledge for each species. Ferret protocols differ from rabbit protocols. Bird doses follow different rules. Reptile recovery depends on temperature.

You do not see these patients every day. When a bearded dragon comes in with metabolic bone disease, you want your notes from the last reptile case. When a client brings a sugar glider, you want the care sheet you wrote last year.

Keep species sections in the reference file. Examples are your ferret adrenal disease workup, the rabbit GI stasis plan that works in your hands, and your budgie crop-feeding technique. The notes are ready when the case arrives, and you do not start the research from scratch.

## Client communication, from the chart

Clients differ. One owner calls twice a week about an anxious terrier and needs reassurance more than medical changes. Another wants lab values, trends, and citations. A household with four dogs mixes up which medication goes to which dog.

Record these communication needs in the client record in the chart. When you need a discharge summary or an update, give Claude the relevant notes. Ask for a draft in that client's format. That can be a written medication schedule per dog, a short reassurance update, or a data-rich summary. If a client needs instructions in Spanish, have a fluent person review the translation before it goes home.

Record what you already explained. Last month you spent 20 minutes on why a normal blood panel did not rule out the owner's concern. A note in the chart keeps you from repeating that talk or contradicting it.

## Surgical and procedural notes

You develop preferences in technique. You have a suture pattern for gastropexies and an approach to dental extractions in cats with resorptive lesions. You have an anesthetic protocol that works best for brachycephalic breeds in your hands.

These are not textbook standards. They are methods you refined from outcomes. When you have not done a procedure in six months, you want your notes, not a generic tutorial. Keep them in the reference file.

Post-operative history belongs to each patient. One schnauzer needed extra pain control after a spay. One cat reacted to the standard post-dental antibiotic. One dog pulled its sutures despite an e-collar. Record it in the chart, where the next surgical plan for that patient will find it.

## Chronic case management

Diabetes regulation takes months. Kidney disease management is ongoing. Allergy work needs long trials. You follow trends across visits weeks or months apart.

Charts capture lab values and prescriptions. They do not always capture small observations. The dog drinks more on hot days. The cat eats less when the household is busy. The insulin dose worked until the patient gained weight. Record those observations in the chart as they come up.

Before a recheck, export the case history and ask Claude for a one-page trend summary. It lists values, dose changes, and the owner's observations, each with its date. You walk in with the full picture, not only the latest glucose curve. You verify the summary against the chart before you rely on it.

## Your practice knowledge

Ten years of practice teach you what vet school did not. You know the breed problems you have seen firsthand and the regional parasites and their seasons. You know what affects client compliance, and what each local emergency clinic can handle.

That knowledge lives in your head, because there has been no practical place to keep it. Written notes go stale. Digital files get lost in folders. Reference software does not include your experience.

The reference file becomes that place. Add each fact as you learn it, with a date. Keep it in plain text so it outlasts any one tool. [Plain text as the durable layer](/articles/plain-text-ai-memory) explains why.

## Set up the reference file

1. Set up Claude Code, which needs a paid Claude plan or an Anthropic API account.
2. Create a folder and a `CLAUDE.md` file at its root. If you sync the folder with Dropbox, iCloud, or Google Drive, check your practice policy first, because sync makes another copy.
3. Add sections for protocols, drug observations, species notes, techniques, and local resources.
4. Add the standing rules: cite the file or the formulary, and never supply a dose from general knowledge.
5. Test with three requests: a protocol, a client handout, and a species question. Check each answer against your notes.

When you learn something new, such as an interaction, a protocol change, or a new local resource, update the file. The next session reads it. When an answer is wrong, fix the file so the [correction reaches every later answer](/articles/corrections-become-system-memory).
